Summary

This position is for the Lead-Free Families Office, which is in the Dept. of Community Health & Wellness. It is a full-time Mon-Fri. 8am-4:30pm with half hour lunch. Occasional evening and weekend hours.

Job Description

POSITION SUMMARY:

Coordinate the lead program and support community health improvement efforts within the organization and throughout the service area.

HOURS: Monday-Friday 8am-4:30pm (occasional evening/weekend hours)

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Coordinate daily operations of the lead program and ensure all requirements are met.

  • Maintain relationships with a variety of community agencies and facilitate communication between these agencies and patients, families and staff.

  • Set program goals, evaluate processes, and report program outcomes using Lean Management methods.

  • Assist in determining eligibility of families enrolled in the lead program and Healthy Homes program.

  • Outreach to target populations using appropriate age and culturally competent strategies.

  • Meet all reporting requirements.

  • Assist manager in tracking budget expenses and revenue related to assigned priorities as appropriate.

  • Serve as a role model for wellness to colleagues, staff and community at large.

  • Provides technical assistance for the data management software including creating and modifying data touchpoints, bulk uploads, and monitoring for quality control.

  • Create, update, and maintain referral systems to ensure participant enrollment.

  • Receives all payments from various participants and ensures the payment is processed through the appropriate system.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Work collaboratively with team members on projects, continuous quality improvement, and team education.

  • Work as a team member with emphasis on ensuring internal/external customer needs met.

  • Assists with grant writing

  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • Bachelor’s Degree in Business Administration, Public Health.

  • Three (3) years of clerical or clinical experience in a health care setting.

  • Three (3) years of patient outreach experience via telephone, electronic, or written communication.

  • Three (3) years of Microsoft Office experience including basic Excel navigation.

  • Skilled at handling stressful situations and providing a high level of customer service in a calm and professional manner.

PREFERRED QUALIFICATIONS:

  • Bachelor’s degree in public health or health related field.

  • Experience utilizing Epic as an electronic medical record.
